Can I use these services?
Council gives services and support to:
- older people who are frail or at risk
- younger people with a disability who require support, and
- their carers.
Council wants to help people to live independently at home.
The assessment process
The State and Federal Governments, together with Council, fund the Home and Community Care (HACC) program. There is a requirement that people accessing these services are assessed at home to work out specific eligibility and support needs.
Once Council accepts a referral, a home-based assessment is arranged. The Council Assessment Officer visits your home and discusses your care needs. They explain Council's services in more detail.
Referrals
Residents can refer themselves for services or a referral can be made by a relative or friend, doctor or other service provider, with the person's consent.
A referral or enquiry can be a phone call or fax to Council - ask to speak to the Intake Officer.
Service providers need to use the SCTT Tool to make a referral.
Online referrals
You can also do an online referral, called an ‘e-referral’, at the Connecting Care website.
- Register at the Connecting Care website
- Login with your username and password.
- Select “Moreland City Council” from the list of Service Providers.
- Select the type of service you would like, for example, Home maintenance or Food services.
- Fill in the rest of the online form and submit.
